# Client record

> The contact as a clinical record — identity, risk, a timeline of every session and interaction, scored measure trends, and your progress notes.

Your client list is also your clinical record. Open any contact and you're looking at a record of your work with that client: their identity information, a risk indicator when one has been raised, a day-by-day activity timeline, scored measure history charted over time, and the progress notes written after each session.
