Opening a contact in Rivet opens a record. At the top sits a strip of identity information — name, date of birth, phone number, emergency contact, referral source, and consent notes. Below that, every interaction with this client appears on a timeline grouped by day: voicemails, text conversations, calls, video sessions, completed assessments, and progress notes. Alongside the timeline, a set of small charts shows each scored measure over time. If a recent clinical assessment raised a risk flag, a banner stays at the top of the record until a newer assessment updates it. On desktop, the client list and the record sit side by side. On mobile, open a contact to see the full record; individual entries open in a sheet above it. ## Identity information The identity strip at the top of the record holds: * **Name and date of birth** * **Phone number** — the number associated with this client's calls and texts on your Rivet line * **Emergency contact** * **Referral source** * **Consent notes** — a free-text field for noting when and how consent was documented Edit any of these fields directly on the record. This information is stored in Rivet alongside the session history; it doesn't sync to or from your existing practice-management software. ## The risk banner When a client completes a PHQ-9 with a non-zero Item 9, or when you administer a C-SSRS that raises items 4, 5, or 6, Rivet marks the contact as elevated risk and shows a banner at the top of the record. The banner shows which measure and which response triggered it. The flag stays visible every time you open this record until a newer administration of the same measure produces a different reading. Rivet doesn't send automatic notifications or escalations when a risk flag appears. The flag is for you. Every interaction Rivet has with this client appears there: * **Voicemails** — tap to read the transcript or listen to the recording * **Text conversations** — the message thread for this client's phone number * **Video sessions** — the session record, including which assessments ran and whether a progress note was written * **Progress notes** — attached to the session that generated them; unsigned notes show a count on the client's row in the contact list Tap any entry to see the detail. A video session entry opens the session record, from which you can view completed assessments (with the full question-by-question responses) or open the progress note. ## The outcomes strip Every scored measure this client has completed appears as a small chart — one per instrument — showing the score at each administration over time. Four PHQ-9 administrations over three months show as four points on the line. A first-ever GAD-7 shows a single point. The charts sit alongside the timeline on desktop; on mobile they appear in the record above the timeline entries. The same data lives in your EHR when you paste score blocks into your session notes.
