# Notes + documentation

> SOAP, DAP, and treatment-plan notes you author in-session — saved to your client's record and ready to export.

Rivet's session-note system is built around one rule: no AI scribe, no session
recording. You write the note; the templates already on screen pre-fill the
structured clinical data. Notes save automatically to the client's record as
you type — drafts until you sign them, locked after that. Export to your EHR
by copy-paste or PDF whenever you're ready.
