# Measurement-based care

> Validated measures administered in session, scored live, and ready for your record.

Measurement-based care is the practice of administering validated outcome
measures consistently across treatment and using the results to inform what
you do next. Rivet handles the in-session piece: your client fills the
measure on their device during the session, scores compute live as they
answer, and severity bands surface inline. The completed measure copies
into your notes in one tap.
