# Tracking measures over time

> Where to see score-over-time trends on the client record, and how to carry measures through your chart.

The whole reason for administering the same measure consistently is to see
the trajectory. Rivet shows that trajectory on the client record, and every
completed measure is also one tap away from your EHR.

## Trends on the client record

Open any client's record and scroll to the outcomes strip — a row of small
charts, one per measure you've administered for that client. Each chart
plots the score over time. PHQ-9 scores from intake, two weeks in, and four
weeks in sit on the same line so the direction is visible at a glance.

The same applies to GAD-7, PCL-5, and every other scored measure you use in
session. The strip updates each time a new measure is completed — no manual
entry, no copy-paste.

## Per-session capture: getting scores into your EHR

Every time you open a measure during a session, the completed result lands
in your record by way of two mechanisms.

**Score in your session note.** When you click "Copy for session notes"
or "Download" on the completed measure, the output includes the
score block — subscale labels, scores, severity bands, any cutoffs that
fired. This pastes directly into your EHR free-text field or saves as a
PDF you can attach. Your session-by-session record of the score lives
where your record lives.

**Autofill into SOAP / DAP notes.** If you open a SOAP or DAP note template
later in the same session, the score block from a separately-administered
measure earlier in the same session pulls into the right section. You
don't re-type the PHQ-9 score into the Objective section — it lands there
as a pre-filled draft you edit before exporting.

The result: a session note that includes the measure result is one tap
away. For practitioners who carry the longitudinal chart in Jane, Owl, or
a paper file, the score block pastes directly in.

## Practical patterns for your EHR

For practitioners who also want measures recorded in Jane or Owl, a few patterns from those already running this loop:

**Use a consistent label.** When you paste the score block into your note,
keep the label format identical across sessions ("PHQ-9 = 14, moderate")
so your eye picks up the change at a glance when scanning past notes.

**Pin the date.** The "Copy for session notes" block includes the
measure title and the session timestamp. Don't strip the timestamp —
that's what lets you read the trajectory weeks later.

**Standardize the cadence.** Intake. Every 2–4 sessions. Discharge. The
trajectory is only legible if the measurements are at a steady rhythm.

**Attach PDFs for the audit trail.** "Copy for session notes" gives you
the structured text; "Download" gives you the verbatim form including
every item the client answered. Some clinicians want the structured note
text in the chart and the PDF attached to the visit record. Both are
generated by the same click, so it's a small extra step.
