Opening a note
From inside a video session, tap or click Write note to open the progress-note editor. The note is already linked to this session and this client. Choose DAP or SOAP — the choice sets the section structure, and you can switch formats before you’ve written anything. If the session ends before you open a note, you can start one from the client’s record. Tap the session entry in the timeline, then tap Write note.Writing the note
The editor has sections corresponding to your chosen format. DAP: Data, Assessment, Plan. SOAP: Subjective, Objective, Assessment, Plan. Each section is a free-text field. Write as you would in any other documentation context — there’s no required structure inside a section, only the headings. The note saves automatically as you type. Close the editor and the draft is waiting the next time you open it.The unsigned-note indicator
Any client with one or more saved but unsigned drafts shows a count on their row in the contact list — for example, 2 unsigned notes. This keeps documentation that still needs your attention visible without opening each record individually.Signing the note
When the note is ready, tap Sign note in the editor. Signing marks the note as finalized and locks the body. The signed note is labeled with the date and time of signing. For practitioners who also sign notes in their EHR — Jane, Owl, or another system — Rivet’s signature and your EHR’s signature are separate steps. Rivet’s is a record-keeping step within the Rivet system; your EHR’s signature is the one your college expects in your system of record. If you need to correct a signed note, use an addendum. Open the signed note and tap Add addendum. Write the correction or additional context. The addendum is dated and appended — the original text stays intact and unchanged, and the correction appears below it as a separate dated entry. This is the standard way to handle corrections to a clinical record: the original entry doesn’t disappear, and the correction is visible with its own date.
